Participants at Thome PACE (Program of All-Inclusive Care for the Elderly) may receive medical and supportive services in our Day Center located in Jackson, Michigan. We also offer a variety of services that can be provided in the comfort of your own home. We have relationships with many specialists and providers in the community to ensure that participants get the care they need.
