Are you or a loved one recovering after an illness or injury? Your doctor may prescribe follow-up care. The skilled team of Porter Hills Home Health Care registered nurses and licensed therapists will work with your doctor to tailor a health care plan that addresses your individual needs – all in the comfort of your own home.
A Registered Nurse or Therapist will conduct a preliminary assessment of your healthcare needs. In coordination with your personal physician and family, we can help you with a range of services at home, such as:
- Treating an illness, such as pneumonia or an infection
- Managing a disease, such as diabetes
- Providing specialized treatments and procedures, e.g., for Parkinson’s disease or respiratory issues
- Treating a wound
- Offering health and nutritional education
- Therapy following illness, surgery, or injury
Licensed Medical Social Workers (LMSW) advise you on ways to maintain your independence as you age and if your health changes, so you can remain living safely in the home you love. Our professionals teach coping skills, stress management techniques, and assist with identifying and securing resources in your community to meet your unique needs.

Home health aides make it possible for you to continue receiving in-home health care by helping with such activities as bathing, shampooing, grooming, exercises, ambulation and meal preparation, on a short-term basis, all under the supervision of a registered nurse or therapist.
HeartBeats is a special cardiac care program of Porter Hills Home Health Care. Our goal is to help cardiac patients reach their best health in spite of their heart condition(s). We hope to achieve this goal by providing patients with the skills necessary to manage their condition at home and by keeping them out of the hospital. The HeartBeats program focuses on the care of patients with one or more of the following four heart conditions: heart failure (CHF), atrial fibrillation (an abnormal heart rhythm), post-heart attack (myocardial infarction or MI), post-bypass surgery (Coronary Artery Bypass Graft or CABG). Our clinicians take the time to work with our patients in order to help them better understand their condition, medications, diet, activity level, and other lifestyle changes they can make to positively impact their health. In addition, our clinicians have many tools they can use to better care for our cardiac patients. We can provide 24-hour telemonitoring in the home. Patients can check their blood pressure, pulse, oxygen level, and weight and a team member monitors the results on a daily basis.
